Laura D. Straus, Ph.D., of San Francisco VA Medical Center and colleagues used data from a diverse cohort of individuals seen in emergency departments to improve recovery after exposure to traumatic stress by using wrist wearables. We investigated whether the device could provide useful biomarkers. After exposure to traumatic stress. The participant wore a wrist-worn device for her eight weeks and completed a battery of neuropsychiatric symptom assessments. A total of 3,040 participants met study criteria, and 2,021 were included in the analysis. Participants were divided into two groups of 1,010 her for biomarker identification and validation.

The researchers found that eight wrist wearable device biomarkers representing symptoms of post-traumatic neuropsychiatric sequelae crossed significant thresholds in a derived cohort. Of these, a decrease in 24-h activity variance was associated with an increase in pain severity. An association was observed between changes in pain over time and changes in six resting activity measures. Changes in the number of transitions between sleep and wakefulness over time were associated with changes in pain, sleep, and anxiety. Individuals with good pain, sleep, and anxiety recovery were identified with high predictive value using simple cutoffs for these biomarkers (positive predictive value, 0.85, 0.63, and 0.76, respectively).

“In the future, such biomarkers may help identify trauma survivors who deserve further evaluation for adverse trauma outcomes, especially in vulnerable populations,” the authors wrote.
